Medicine and Food Prescription Refills

In our ongoing effort to make your pet's health care as convenient and easy as possible, you can now request a refill for your pet's medicine or food prescription by submitting the following form. Please be sure to fill in all the requested information.

Since your pet's prescription will need to be reviewed by one of our veterinarians, please give us at least 24 hour notice that you are in need of a refill.  If you are out, please call the office directly at 860-779-0608. You can also visit our dedicated online store through our website, www.BrooklynVetHospital.com, and click on the Online Store link on the Home page to request medicine and food prescriptions.

Please let us know the best way to reach you.  We will notify you via text, email or phone when your pet's prescription is approved and ready to be picked up. We will also inform you of the total cost of the prescription, and will request a credit card number by phone at that time.  If you would prefer to have the prescription mailed to you, please mention this information in the additional information area.
